Good Faith Estimate

Your rights as a self-pay patient

Your Right to an Estimate

Under the federal No Surprises Act, patients who do not have health insurance or who choose not to use insurance have the right to receive a Good Faith Estimate of the expected cost of their care.

When You Will Receive It

If you schedule a service at least 3 business days in advance, Naiman Health will provide a Good Faith Estimate of the expected charges for the scheduled services. You may also request a Good Faith Estimate before scheduling care.

What the Estimate Means

A Good Faith Estimate is not a bill. It reflects the charges that are reasonably expected based on the information available at the time the estimate is prepared. Additional services or costs that could not reasonably be anticipated may not be included.

If Your Bill Is Higher Than Expected

If you receive a bill from Naiman Health that is $400 or more above the amount listed on your Good Faith Estimate, you may have the right to use the federal patient-provider dispute resolution process.
For additional information about your rights under the No Surprises Act, you may visit the Centers for Medicare & Medicaid Services website or call the No Surprises Help Desk at 1-800-985-3059.

Naiman Health Is Self-Pay

Naiman Health is a self-pay practice and does not bill health insurance for visits or memberships.
